We welcome referrals for our supported living services from a wide range of sources, including the person themselves, family members, social workers, community mental health teams, hospital discharge teams, local authority commissioners, and other health and social care professionals. If you are exploring supported living options for yourself or someone you know, we are here to help.

Who Can Make a Referral

Self-Referral

You can refer yourself directly to our service. We welcome enquiries from individuals who are interested in supported living and want to understand what we can offer.

Family and Friends

Family members, carers, and friends can make a referral on behalf of a person, with their consent.

Social Workers and Care Managers

Local authority social workers and care managers can refer people who have been assessed as needing supported living services.

Community Mental Health Teams

CMHTs can refer people with mental health needs who would benefit from supported living.

Hospital Discharge Teams

Hospitals can refer people who are ready for discharge but need supported living to return safely to the community.

Commissioners

Local authority commissioners can refer individuals as part of commissioned care packages.

Other Professionals

GPs, advocacy services, housing providers, and other health and social care professionals are welcome to make referrals.

Our Referral Process

The following six values underpin every aspect of our service and are embedded in our recruitment, training, supervision, and quality assurance:

Initial Contact

Contact us by telephone on 0151 453 5560 or by email at info@winsomecare.co.uk. We will have an initial conversation to understand the person's needs, circumstances, and what they are looking for from a supported living service. There is no obligation at this stage.

Referral Information

If the referral is progressing, we will ask for some basic information about the person, including their name, date of birth, current living situation, the support they need, any relevant assessments or reports, and any specific preferences or requirements. This information helps us determine whether we are the right provider.

Pre-Support Assessment

Our Registered Manager or a qualified senior member of staff will conduct a comprehensive, face-to-face pre-support assessment. This covers the person's care and support needs, their goals and aspirations, risk factors, communication needs, cultural preferences, and the suitability of any proposed accommodation. We only accept referrals where we are confident we can deliver safe, effective, caring, responsive, and well-led support.

Support Planning

Once a referral is accepted, we co-produce a detailed, person-centred support plan with the individual, their family or representatives, and relevant professionals. This plan forms the foundation of the person's care and is a living document reviewed regularly.

Commencement of Support

We carefully match support workers to the person based on skills, experience, personality, and compatibility. Support begins in line with the agreed plan, with close monitoring during the initial weeks to ensure everything is working well.

What happens after you refer

What Happens After You Refer

We aim to acknowledge all referrals within one working day and to arrange an initial discussion within three working days. The pre-support assessment will be arranged at a time and place that suits the person, and we will keep the referrer informed of progress throughout the process. If we determine that we are unable to meet the person's needs, we will explain why and help signpost them to a more appropriate service.
