Eligibility & Referrals
Who is eligible for our services?
Our support is available to adults aged 18 and over who have care and support needs. We specialise in supporting individuals with diagnosed mental health conditions, learning disabilities, autism, or other long-term conditions. We also work with people who have additional needs such as complex behaviours, or dual diagnoses.
Eligibility is determined through a social care assessment completed by your local authority. We work with people who need help to achieve everyday outcomes, maintain their wellbeing, and live safely and independently in their community.
At Winsome Care, we offer safe, comfortable home care and supported living services with staff available 24/7— including waking night support when required. Each person receives a personalised care and support plan tailored to their needs, preferences, and goals. Our on-call system ensures senior staff are always accessible, providing reassurance day and night.
Whether you need emergency placement, short-term support, respite, or a long-term home, we provide a stable, supportive setting that promotes independence, wellbeing, and choice.
Who can make a referral?
Referrals are welcome from Local Authorities, NHS teams, Community Mental Health Services, and low- to medium-secure hospital services for individuals preparing to transition back into the community.
We also accept self-referrals, giving individuals, families, and carers the option to contact us directly for support.
